Our Mission in Portland

Riverfront Wellness Center is a nonprofit community health hub located along Portland's riverfront, dedicated to eliminating health disparities in the Lents, Cully, and Rockwood neighborhoods. We provide free and low-cost preventive care, behavioral health, nutrition education, and youth wellness programs to over 4,200 residents annually.

Equity-Centered Approach

We meet communities where they are—offering multilingual services, trauma-informed care, and mobile outreach to ensure everyone has access to quality health resources regardless of income, language, or documentation status.

Community Trust

Built on 7 years of deep neighborhood relationships, our staff are 78% bilingual and 65% from the communities we serve. Trust is our foundation for lasting health behavior change.

Data-Driven Results

Every program is measured. In 2024 we achieved a 34% reduction in ER visits among enrolled chronic disease patients and a 41% increase in childhood vaccination rates in target zip codes.

Local Services & Programs

Community Health Screenings

Free monthly blood pressure, diabetes, and cancer screenings at 6 neighborhood locations. 1,850 screenings delivered in FY2024.

Behavioral Health & Counseling

Culturally responsive therapy, peer support groups, and crisis navigation in English, Spanish, Vietnamese, and Somali.

Nutrition & Food Security

Weekly produce markets, cooking classes, and a mobile food pharmacy serving 320 families/month with chronic disease management support.

Youth Wellness Initiative

After-school fitness, mindfulness, and health leadership programs for 240 middle and high school students in East Portland schools.

Maternal & Family Health

Prenatal classes, home visiting, and postpartum support groups in partnership with OHSU and local doulas. 89 births supported in 2024.

Health Navigation & Enrollment

Bilingual enrollment assistance for Medicaid, SNAP, and housing resources. 1,120 successful enrollments last year.

Regional Health Equity Grant Application Support

Riverfront Wellness Center respectfully requests $250,000 over 24 months to expand our mobile health equity outreach, double our youth wellness cohorts, and launch a new community health worker training pipeline serving East Portland’s most underserved populations.
